Bethany Medical Clinic


The Bethany Medical Clinic (BMC) became a service of Harrison County Community Hospital on May 3, 2010. Our practice is comprised of Dr. Chad Lambert, Tamula “Tammy” Maxwell, Certified Physician Assistant, and Cephas Paniamogan, Certified Family Nurse Practitioner.

Using a team approach, BMC provides outpatient primary care services and basic laboratory services as a department of Harrison County Community Hospital.

As a rural health clinic, we are dedicated to improving access to primary care in our rural location. We have a working relationship with Crestview and Bethany Care Center, allowing local patients convenient access to our area’s healthcare resources.

Saturday Morning Clinic: Patient check-in 8:30 to 11:15 a.m. (No appointment required)

Registration

As a patient, you can now print and complete your Patient Information Form in advance so that you may bring it with you at the time of your appointment.

Our healthcare providers are committed to the best possible outcome for their patients. The more they know about your health history, the better able they are to provide the high quality healthcare you deserve and have come to expect from Harrison County Community Hospital and the Bethany Medical Clinic.

If you’re anticipating a visit to BMC, complete the following form and fax, mail, or bring it to BMC with a list of your previous healthcare providers, or send it directly to those providers, so that we may retrieve your health history prior to your appointment.

Prescription Medication Refills

For prescription medication refills, please contact your local pharmacy.  

Plan to allow your pharmacy three business days for processing your refills.
