HCCH Medical Clinic has been a service of Harrison County Community Hospital since May 3, 2010. As a Rural Health Clinic and a Safety-Net Clinic, we are dedicated to improving access to primary health care for the residents of our rural communities, including individuals with limited English proficiency.


Our Services

Our physicians, physician assistant, and a family nurse practitioners provide comprehensive primary care services, including adult and pediatric care, preventative care, evaluation and treatment of acute and chronic illnesses, obstetrics, wellness checks, men's and women's health care, on-site lab services, immunizations, school physicals, DOT physicals, drug screenings, and breathalyzer testing (DOT and non-DOT).

Our Providers

  Chad Lambert, DO

Dr. Lambert, a native of Worth County, Mo., earned a B.S. in Pre-Professional Zoology from Northwest Missouri State University. He earned a PhD in Molecular Microbiology and Immunology from the University of Missouri-Columbia and his medical degree from the Kansas City University of Medicine and Bioscience. He joined the Bethany Medical Clinic staff in August 2012.
  Gregory Dean, MD

Dr. Gregory Dean, a native of Buhler, Kan., received his undergraduate degree from MidAmerica Nazarene University and his medical degree from the University of Kansas School of Medicine. He has been practicing medicine in Bethany since 2005.  

Kimberly Baker, MD    

Dr. Kimberly Baker grew up in Harrison County. She received her Bachelor of Science in Biology with a minor in Chemistry. She then attended the University of Kansas School of Medicine and graduated with an MD. Dr. Baker then went onto Mercy Medical Center Family Medicine Residency in Des Moines, IA, and was a part of a Rural Training Track with Dr. Wells in Corydon, IA. She then moved to Cannon AFB to work with the United States Air Force for four years. Dr. Baker enjoys treating a variety of medical conditions. She offers Battlefield Acupuncture, Dry Needling, Occipital Nerve Blocks, Sphenopalatine Ganglion Blocks, and Botox for migraines. Dr. Baker focuses on whole-person care regarding chronic pain and migraines. She also performs IUDs and Nexplanon for birth control. Dr. Baker resides in Bethany with her husband and three children. 
  Zachery Mahler, MD    

Dr. Zach Mahler grew up in Lincoln, NE, where he graduated from the University of Nebraska Lincoln with a Bachelor of Science in Biology with a minor in Humanities in Medicine. He then attended the University of Nebraska Medical Center and received an MD. Dr. Mahler went on to his family medicine residency back in Lincoln, NE, at Lincoln Medical Education Partnership. He graduated this June and joined the HCCH Medical Clinic, where he will be doing pediatrics, including newborns, preventative care, Colonoscopies, EGDs, asthma, and many others. Outside of work, Dr. Mahler enjoys farming or playing competitive Ultimate Frisbee. He resides near Gallatin with his wife and two daughters.

Ashley Harding, FNP-C, CEN

Ashley Harding earned a master's degree in Nursing from Graceland University and is board certified as a family nurse practitioner. She is also a certified emergency nurse. Before joining the Bethany Medical Clinic staff in February 2015, she worked in medical-surgical nursing, mental health, corrections, and emergency nursing. She resides near Grant City with her husband, Jared.
   Kelly Clark, MSN, APRN, FNP-C

Kelly is a native of Harrison County and a graduate of South Harrison High School. Her undergraduate studies include a Bachelor of Science from the University of Missouri in Nutrition and Fitness, as well as a Bachelor of Science in Nursing from William Jewell College. She received her Master’s Degree in Nursing with a Family Nurse Practitioner focus from Saint Luke’s College of Health Sciences. She has a variety of nursing experience including emergency nursing, labor and delivery, newborn nursery, and medical-surgical nursing. Kelly resides with her family on their farm near New Hampton.
 Cephas Paniamogan, FNP-C Cephas Paniamogan, FNP-C 

Cephas Paniamogan was born and raised in Ridgeway, Mo., and has been employed by HCCH since 1996. He received his Nurse Practitioner License in December 2010 and joined the Bethany Medical Clinic's provider staff in June 2011. He and his wife, Ingrid, an HCCH physical therapist, have three children.
   Tammy Maxwell, PA-C, MS, RN, CEN, CVRN

Tammy Maxwell grew up in Harrison County and has worked in local health care for more than 20 years. She received her master's degree in Advanced Physician Assistant Studies from A.T. Still University of Health/Sciences/Arizona School of Health Sciences and her Physician Assistant Certification from the University of North Dakota School of Medicine at Grand Forks, ND. She is also Certified in Emergency Nursing (CEN) and Cardiovascular Nursing (CVRN-BC Level 1)


Appointment Hours

Monday–Friday, 7:00 am-5:00 pm

Bethany Convenient Care Clinic offers weekday evening and Saturday hours. No appointment is required.
